The “Rules Related to Madhya Pradesh State Disease Assistance Fund” scheme was launched by the Department of Public Health and Family Welfare, Government of Madhya Pradesh. The scheme aims to provide financial assistance for major surgical and medical treatments to Below Poverty Line (BPL) patients suffering from life-threatening diseases.

Scheme Implementation:

The scheme is implemented by the State Illness Assistance Fund (SIAF) Management Committee, supported by District Collectors, Civil Surgeons, and designated medical institutions.

Benefits

  • - Financial assistance ranging from ₹25,000to ₹2,00,000for major surgeries or medical interventions. NOTE: 1. The SIF assistance amount cheque is provided directly to the Hospital/Institution where the approved case (patient) is referred. 1. The grant will be permissible for the recognized hospitals within the state & outside the state. 1. For cancer cases, the grant will be given to the JLN Cancer Hospital, Bhopal
  • Financial assistance ranging from ₹25,000 to ₹2,00,000 for major surgeries or medical interventions.

NOTE:

  1. The SIF assistance amount cheque is provided directly to the Hospital/Institution where the approved case (patient) is referred.
  2. The grant will be permissible for the recognized hospitals within the state & outside the state.

Eligibility

  1. The applicant must be a domicile of Madhya Pradesh.
  2. The applicant must belong to a Below Poverty Line (BPL) family.
  3. The applicant must be suffering from a disease listed among the approved illnesses under the scheme.1. The hospital must be a recognized institution within or outside Madhya Pradesh.
  4. Only one eligible family member can receive assistance once.

Application Process

Offline

Step 1:**** The interested applicant downloads the application form from the official website.
Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).
Step 3: Submit the duly filled and signed application form along with the documents to the respective District Collector, addressed to the Director, Public Health and Family Welfare.
