To apply, submit the application form along with the required attachments to BLOCK NO-19/1, Dr. JIVARAJ MAHETA BHAVAN, GANDHINAGAR, GUJARAT- 382010. Required attachments include:

20 copies of the script
A copy certified by a Chartered Accountant (C.A.)
Audited accounts by C.A.
Attested copy of PAN Card
Certificate of Film Production
Certificate of Sound Recording and receipt
Self-attested copy of Censor Certificate
Show-wise sold tickets form from the distributor.
