The eligible applicants can obtain the application form from the office of the Assistant Social Welfare Officer or download from the departmental website (https://socialwelfare.uk.gov.in). - After that, the duly filled application form must be submitted along with the following documents: Aadhaar card of the family member/applicant, mobile number, attested photograph, bank account passbook linked/seeded with Aadhaar, BPL certificate, copy of family register, death certificate of the earning member, and Aadhaar card of the earning member.
  • The application form along with the necessary documents are submitted to the office of the Assistant Social Welfare Officer. The application is sent to the Government of India from the district level and if the documents are found to be correct, the amount is paid directly into the beneficiary's account.
