For Beneficiary (Person who underwent Sterilization)
  • The applicant must be a person who has undergone sterilization operation under family planning program.
  • The applicant must have undergone sterilization operation at an authorized health facility.
  • The applicant must be able to provide documentary evidence of the sterilization procedure.
For Claimant (Family Member or Legal Representative)
  • The applicant must be a legal claimant or family member of the person who underwent sterilization (in case of death or incapacitation).
  • The applicant should have legal authority to represent the beneficiary in case of death or inability to claim.

Application Process

Offline

Step 1: Initial Reporting and Documentation

  • Visit the health facility where sterilization was conducted and report the case of death, failure, or complication to the Medical Superintendent or Health Officer.
  • Begin collecting all required documents including original sterilization certificates, case papers, identity documents, and bank details from relevant sources.

Step 2: Form Completion and Medical Certification

  • Complete Part A of the Claim Form cum Medical Certificate with accurate personal details, sterilization operation information, and nature of claim.- Submit all medical records and completed Part A to the Chief Medical Officer or equivalent designated officer at district level for medical certification.

Step 3: Application Submission

  • Collect the medically certified claim form (Part A and Part B completed) from the certifying officer.

  • Submit the complete application with all required original documents and photocopies to the respective health facility where sterilization was performed.
    Step 4: Follow-up and Verification

  • Maintain regular contact with the health facility to track progress as the case moves through District Indemnity Scheme Committee review.

  • Provide any additional information or documentation if requested during the committee verification process.

Step 5: Payment Processing

  • The District Committee forwards the verified case to the state office (State Health Society) for final approval and processing.
  • Upon state approval, compensation amount is disbursed directly to the beneficiary's bank account as specified in the claim form.
