The “Various Awards to Individuals/Organizations who have made Outstanding Contributions in the Field of Social Welfare” scheme was launched by the Social Justice and Empowerment Department, Government of Gujarat. Eligibility
- The applicant should be either an individual or an organization.
- If the applicant is an individual, they should be a resident of Gujarat; if an organization, it should operate within Gujarat.
- The applicant should have made outstanding contributions in the field of social welfare.
- The contributions should specifically benefit or directly impact the Scheduled Caste (SC) community in Gujarat.

Application Process
Offline
Step 1: The interested applicant should visit (during office hours) office of the Social Justice and Empowerment Department and request a hard copy of the prescribed format of the application form from the staff exclusively entrusted to issue and collect filled-in applications.
Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attested, if required).
Step 3: Submit the duly filled and signed application form along with the documents to the office of the Social Justice and Empowerment Department.
