• Proposed name of the society
• Type of society (choose from primary/central/top)
• Business of the society
• Liability (choose between limited/unlimited)
• In case of limited liability, details of member liabilities
• Class and subclass of the proposed society (select from options available in the provided Excel sheet)
• Total number of applicant members
• List of society members (to be uploaded in the provided proforma)
• If there is a member committee, details of the authorized signatory
• List of weaker sections in case of service cooperative societies
• Details of collected entrance fee
