The scheme "Mukhyamantri Bal Hridaya Upchar Yojana" was launched by the Public Health and Family Welfare Department, Government of Madhya Pradesh. The scheme aims to provide free treatment and surgery to children aged 0 to 15 years who are suffering from identified heart diseases. The scheme provides financial assistance ranging from ₹25,000/- to ₹1,00,000/- for treatment, covering 07 identified heart diseases in children. Eligibility

  • The applicant must be a resident of Madhya Pradesh.
  • The applicant must be a member of an Economically Weaker Family or a Below Poverty Line (BPL) family.
  • The applicant must ensure their name is recorded in the family roster and they hold a ration card.
  • The intended beneficiary (child) must be aged 0 to 15 years.
  • The intended beneficiary (child) must be suffering from one of the identified heart diseases.

Exclusions

The beneficiary is not eligible for assistance if treatment for the sanctioned disease is sought prior to the approval date.

Online

Registration

Step 1: Visit the Official Website of "Madhya Pradesh Lok Sewa Guarantee Portal" and click "नागरिक पंजीयन (Citizen Registration)".
Step 2: In the registration page, provide the following mandatory information: Name, Gender, Mobile No., Email ID.
Step 3: Create a new password and confirm the same. Fill the captcha code. Verify your Mobile Number via OTP. Finally, click "Register".

Application

Step 1: Visit the Official Website of "Madhya Pradesh Lok Sewa Guarantee Portal" and click "लॉग इन (Login)".
Step 2: Login using your registered Email ID and the password created during registration.
Step 3: On the homepage, sort the list of services "By Department". Further, select the Department and the respective scheme that you intend to apply for. On the next page, detailed information about the scheme will be displayed, along with the following resources:

  • Official Guidelines of the Scheme.- Prescribed Format of the Application Form (For Offline Application).- Link to Apply Online (By Self or Through Lok Sewa Kendra / MP Online / CSC).

Step 4: In the scheme's online application form, fill in all the mandatory fields and upload all the mandatory documents in the specified format and size (fields marked with * are mandatory), and click "Submit". Alternatively, you may apply via your nearest Lok Sewa Kendra / MP Online / CSC by making a payment of ₹20/-.

Grievance Redressal / Help & Support (Appeal Mechanism)

The applicant may file an appeal in the following situations:

  1. Non-receipt of the application acknowledgment receipt.
  2. Approved treatment amount is less than the prescribed package/estimate.
  3. Non-resolution within the specified time limit.
  • First Appeal: Must be filed with the Collector. The prescribed time limit for the resolution of the first appeal is 15 working days.- Second Appeal: Must be filed with the Samagra Sanyukt Sanchalak, Swasthya Seva (Integrated Joint Director, Health Services).

Offline

Step 1: The interested applicant should take a printout of the prescribed format of the application form.
Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).
Step 3: Submit the duly filled and signed application form along with the documents, within the prescribed period (if any), to the Chief Medical and Health Officer (CMHO) of their district.
