The form must be accompanied by the following documents: the athlete’s Aadhaar card, bank account number, passport-size photograph, details of the medal won at the competition along with complete certificates, permanent residence certificate of Uttarakhand, and the coach’s qualification documents (either a one-year sports diploma or an equivalent certificate, or a six-week training certificate issued by the National Federation of the concerned sport). In addition, a declaration on a ₹10/- stamp paper must be provided, certifying that the player has been regularly trained by the coach for at least 180 days during the one year preceding the medal achievement. The coach’s Aadhaar card and bank account details must also be submitted.
It is mandatory for medal-winning players of the National School Games to get their sports certificates verified by the School Education Department; for winners of World/All India University Competitions or Championships by the Higher Education Department/University; for winners of Khelo India, Indian Rural Sports Competitions, or National Women’s Sports Festivals by the Youth Welfare Department; and for winners of All India Police Sports Competitions or World Police/Fire Games by the Uttarakhand Police Department.
